Initial assessment

A complete assessment will be performed including a review of the medical and functional condition, as well as an assessment of the physical condition of the individual. Following this evaluation, a report will be produced in which we will propose an intervention plan and recommendations.

Rehabilitation program

The objective of this program will be to accompany patients in their process to regain their health. The ultimate goal of this program is to allow the patient to return to work or the job market successfully and sustainably . This will include individual sessions, as agreed upon with the referrer. The interventions will include education of tools and strategies for the mental health component while promoting the implementation of stimulant physical activities. We will help the person optimize their overall functionality and help them integrate healthier lifestyle habits that will allow them to maintain their personal balance in the future. Follow-ups will be personalized and oriented according to the needs and interests of the patient.

Case Coordination and Reporting

Regular telephone and/or email communication with the referrer is included in the file intake. Progress reports or end of service reports can be written at the request of the referrer.

Services are offered in person at an outdoor location (use of surrounding parks), at home or in private consultation at our private office. Video conferencing is also available.

The services offered are available in the northern suburbs of Montreal (Laval, Lower Laurentians and Lanaudière South) as well as in the eastern part of Montreal. Please note that travel expenses may be charged depending on the distance to be covered.
